    Alone

• Patient can be referred to DTC/DOT Plus site with the help of Government health vehicle or Government Ambulance.
• If patient agrees, you can explore NGOs working for elderly for institutionalization. Explore if tiffin services are available.
• Contact Nikshay Mitra/Sarpanch/ local leaders if they can provide Nutrition kit to the patient or neighbour if they can prepare food and give to the patient; after taking consent from the patient.

    Neglected by family

• Identify family situation/issues and counsel family members to provide care and support to the patient.

    Financial issue

• Make sure DBT is done in patient’s account only. Can link the patient with the following:
– Old age pension scheme.
– Pradhan Mantri Vaya Vandana Scheme
– Indira Gandhi National Old Age Pension Scheme (IGNOAPS)
– National Programme for the Health Care of Elderly (NPHCE)
– Pradhan Mantri Jan Arogya Yojana (PMJAY)
– If patient gives consent then can refer to TG welfare board/ CBO
